Chronic otitis media in urban and rural areas: epidemiological, clinical and therapeutic description

Abstract
Introduction : Chronic otitis media (CMM) is conventionally defined as an inflammatory process of the middle ear cavities that has been evolving for more than 3 months. The aim of this study was to describe the epidemiological, clinical and therapeutic aspects of chronic otitis media in the urban environment.
Material and methods: This is a retrospective study, over a 5-year period, from January 1, 2014 to December 31, 2019. Data were collected from consultation registers and medical records. The parameters studied were analyzed using SPSS version 21 software.
Results: The frequency of CMO in rural and urban areas was 14.1%, with a predominance of urban areas in 60.4%. The mean age of our patients was 28.22 years. Both sexes were involved, with males predominating (51.8%). Reasons for consultation were dominated by otalgia (100%) and otorrhea alone (55.6%). All patients received pus aspiration and medical treatment with ear drops, with or without antibiotics, steroidal anti-inflammatories or analgesics. 10 mastoidectomies (58.8%) were combined with tympanoplasty, and 2 paracenteses (11.7%) with DIABOLO. The major post-treatment complication was anemia.
Conclusion: CMO is a pathology that is wrongly trivialized, but frequently encountered among otological disorders. It can be contracted at any age, with a high prevalence of onset at an early age due to immune immaturity, repeated otitic episodes and poor management of ENT infections, leaving the disease to progress to chronicity.
