Résumé

Introduction : Le sepsis est une cause majeure de morbi-mortalité en soins intensifs, particulièrement dans les régions à ressources limitées ou les retards diagnostics et de prise en charge thérapeutique aggravent souvent le pronostic. En République Démocratique du Congo, les données locales sont rares. L'objectif de cette étude est d'identifier les facteurs associés à la mortalité chez le patient adulte atteint de sepsis en unité de soins intensifs polyvalents de l’hôpital tertiaire HEAL Africa de Goma.


Matériel et méthodes : Notre étude était rétrospective et analytique menée d’avril 2024 à Mars 2025. Tous les patients adultes admis aux soins intensifs pour sepsis ont été inclus selon les critères sepsis-3. Les données anamnestiques, cliniques, biologiques et thérapeutiques ont été analysées. Les facteurs associés à la mortalité ont été identifiés par régression logistique multivariée.


Résultats: Sur 486 admissions, 121 cas de sepsis ont été recensés (24,8 %), dont 104 analysés. L’âge médian était de 52 ans (IQR : 67-35) et le sexe ratio H/F de 1,3. La mortalité était de 30,8 %. Les facteurs indépendamment associés à la mortalité étaient la présence d’au moins deux comorbidités (OR=57,1 ; p=0,001), un score SOFA ≥8 (OR=6,3 ; p=0,028), un délai d’administration des antibiotiques ≥2 heures (OR=5,8 ; p=0,044) et la ventilation mécanique (OR=41,6 ; p p < 0,0001).


Conclusion : Le sepsis chez l’adulte est fréquent en soins intensifs à Goma et associé à une mortalité élevée. Les comorbidités multiples, la gravité initiale et le retard de prise en charge sont les déterminants majeurs du pronostic. La prise en charge adéquate et précoce pourrait réduire la mortalité.
